Primary Care Plus
The Park Farm Medical Centre and Vernon Street Surgery are embarking on an exciting new initiative. We are now part of Primary Care Plus, which is a Community Interest Company (CIC) offering PRE-BOOKED appointments for the patients of the specific member surgeries from 6.30pm to 8.00pm Monday to Friday and at weekends.

These extended access appointments are available for all patients registered at the above Practices and are provided at specific “hub” Practices namely;
HUB - Park Medical Centre (Maine Drive, Chaddesden, Derby, DE21 6LA)
Open Monday to Friday 1800-2000; Saturday and Sunday 0800 – 1200
HUB - Peartree Medical Centre (159 Peartree Road, Derby, DE23 8NQ)
Open Monday to Friday 1800-2000; Saturday and Sunday 0800 – 1200
HUB - Coleman Health Centre (Coleman Street, Derby, DE24 8NH)
Open Monday to Friday 1800-2000
HUB - Derwent Valley Surgery (81 Sitwell Street, Spondon, Derby, DE21 7FH)
Open Saturday and Sunday 0800 – 1200
I am pleased to advise that Park Farm Medical Centre will be opening as the 5th HUB Practice at the end of August 2018 to provide these extended access appointments. We will be open Monday to Friday 1800-2000: Saturday and Sunday 0800 – 1200.
All of the practices involved have a specific number of appointments available to them depending on the size of their practice.
All of these appointments MUST be pre-booked by contacting your usual surgery and asking for an extended access appointment. When you are booked in for an appointment you will be asked the nature of the problem (so we can book you in with the right person) and we also request your consent to share your full record. Without both of these you will not be able to access their appointments. The record sharing allows the clinicians to have full access to your medical records, so they can add any hub consultation, prescriptions, fit note etc straight into your clinical record and have full view of your medical history.
In future we may have appointments available at several of the hubs, so if you work near one of them, it may be easier for you to attend an appointment at one of them on your way home, instead of at your usual practice.
We will be providing 15 minute consultations and have a full range of quality, experienced staff on hand. In the future, we will be offering a variety of services both GP and Nurse Practitioner appointments, blood clinics, smear clinics, long term conditions clinics and dressing clinics.
Remember, we are not contracted to see any walk-in patients during these extended access sessions; only patients that are booked in by their Practice will be seen.
We are excited at Dr P A A Wood & Partners at being involved in this new initiative, resulting in being able to deliver additional services to our patients and extended access to various clinicians.
