Health Checks for patients aged 40 - 74
Diabetic clinic and diabetic education session
Well women and well man clinic
Contraception including coil and implant insertion and removal
Antenatal clinics including post natal checks
Community District Nurses
We are now able to see & treat, or refer patients with minor injuries. The list of recommended injuries to be included are:
Lacerations capable of closure by simple techniques
Superficial Injury of eye
Partial thickness thermal burns or scalds involving broken skin:
◦Not involving the hands, feet, face, neck, genital areas
◦Not over 1 inch diameter
Foreign bodies superficially embedded in tissues, including eyes
An injury that is not amenable to simple domestic first aid, including for example:
Minor Head injuries where there has been no loss of consciousness
Care Co-ordinators are part of the Integrated Community Team employed by Derbyshire Community Health Services and they are based at local GP practices.
Its a free service and they can work with anyone over the age of 18 to help to support and work on your behalf as the link between all the different services and organisations in the community.
Our care co-ordinator is Malcolm Gallon and he works at Park Farm Medical Centre on a Tuesday and Friday.

Derby Urgent Care Centre is open 8.00am – 8.00pm, 365 days a year.
We are here to help when you need advice and treatment for most common injuries and illnesses that are urgent but not life threatening.
There’s no need to book an appointment, just come along to the Derby Urgent Care Centre to be seen by one of our expert urgent care clinicians.
We can help with:
* Sprains and Strains * Minor Burns and Scalds * Minor Head and Eye Injuries * Bites and Stings * Persistent Coughs * Severe Sore Throats * Rashes * Infections * Sudden worsening of long term condition
We are unable to help with:
* Conditions currently being treated by your GP such as on-going/long term problems, prescriptions, sick notes, referrals* Injuries requiring x-ray* Vaccinations * Testing such as blood tests, pregnancy tests, sexual health tests
